Alex took Kelsey to visit Justin at the Kiriakis mansion. When Justin asked Alex if he was okay, Alex admitted that parenting was tough but worth it. Bonnie congratulated Justin on being a grandfather before changing the subject to the lawsuit over her book. Bonnie said she had not intended to plagiarize, but she realized she had, and she apologized. Alex said he understood.

At the pub, Kate noticed that Joy appeared anxious. Joy admitted that she had left Kelsey alone with Alex for the first time. Kate assured Kelsey that Alex would be fine. When Kate mentioned that Stephanie could help, Joy told Kate that Stephanie was in China. Joy shared her concerns about Stephanie’s feelings towards Kelsey. “She wishes my kid didn’t exist,” Joy said. Kate argued that Stephanie needed time to get over the changes in her life. “Maybe she’ll surprise you,” Kate said. Joy admitted she still felt like “the bad guy.”

Kate told Joy about how she had kept a baby secret from the married father in order to protect his family. Joy reminded Kate that Alex had not been married or even with Stephanie. Kate countered that Joy should be grateful that her situation was not worse.

After a tense exchange with Roman, Kate stepped away to talk to Roman privately. By the bar, Roman apologized to Kate for not having trusted her about the lawsuit. Kate thanked Roman, but didn't tell him the truth.

When Alex arrived with Kelsey, they walked Joy out of the pub. Alex told Joy about his successful day with his daughter. Despite Joy’s reservations, Alex said he believed Stephanie was ready to spend time with Kelsey.

Marlena greeted Johnny and Chanel as they arrived at the hospital. While Chanel went to the bathroom, Johnny told Marlena that the biopsy had confirmed that Chanel had breast cancer. Johnny apologized to Marlena for telling her the news on the anniversary of John’s death. Marlena assured Johnny that it was fine and offered her support.

When Chanel returned, Marlena had already left for her office. Johnny confirmed that he had told Marlena the news. Chanel and Johnny went to meet with the oncologist, Dr. George. After a rough start when Dr. George was too clinical and upset Chanel, he softened. Dr. George explained the treatment protocol and confirmed that Chanel would undergo chemotherapy during her pregnancy and hormonal therapy after the pregnancy.

Concerned, Chanel peppered the doctor with questions. When Dr. George was defensive, Johnny interrupted to note that his wife needed to make sure that she was comfortable with her treatment plan. Chanel argued that she was not a statistic, and that she needed her doctor to see her as someone who mattered. When Dr. George claimed to respect his patients, Chanel disagreed.

“I’m scared, and not of the cancer, but of how little you seem to care,” Chanel said. Chanel argued that Dr. George had done nothing to address her concerns about her baby. Dr. George stressed that he cared about his patients, and he advised Chanel not to place higher importance on the internet than his advice. Dr. George told Chanel to calm down and make an appointment to start chemotherapy.

Truth or consequences

When Xander met up with Marlena in her office, she explained that she had been thinking about John. Marlena said that she believed that John would not want her to work with Xander and Kate after how they had treated Johnny. Marlena told Xander that if he did not drop the lawsuit, she would walk away from Basic Black. When Xander asked for reasons, Marlena noted that Xander needed to take responsibility for his actions. Marlena noted that although Xander had made progress in many ways, the lawsuit was a different story.

Marlena told Xander to apologize to Johnny and ask for his forgiveness. Xander argued that Marlena could not force him to tell anyone about what he had admitted to in therapy. Marlena agreed and noted that the only thing she could do would be to stop treating Xander. Xander was shocked that this was Marlena’s red line. Marlena argued that Johnny did not need to deal with a lawsuit, and this was the only way she could help him.

“That sounds an awful lot like extortion to me,” Xander said. Marlena encouraged Xander to break his cycle of abusing his power and then regretting his choice. Frustrated, Xander complained that he did not want to start over with a new therapist. Marlena told Xander that if he did the right thing and dropped the lawsuit, she would work with Kate, and she would continue to treat Xander. Xander argued that Roman would never forgive Kate if he knew what she had done, but Marlena shrugged dismissively. Marlena noted that Roman and Johnny both deserved to know the truth.

Sarah ran into Marlena after she finished her conversation with Xander, and she asked Marlena if she was doing okay on the anniversary of John’s death. Marlena said she was fine, and she was looking forward to a family dinner with her kids. Marlena invited Sarah to join them. Xander listened from nearby as Sarah politely declined the offer. “You’re important to Brady. That makes you important to us,” Marlena said. Sarah promised to think about the offer.

As Johnny and Chanel left their appointment, they ran into Sarah and Marlena in the hallway. Johnny noted that Dr. George’s bedside manner had been a problem. Marlena admitted that Dr. George was known for “not being receptive to his patients’ concerns.” Sarah noted that although Dr. George was a great oncologist, it did not mean he had the best bedside manner. Chanel refused to be pressured into treatment by an “egotistical doctor.”

A pain that is going to linger

Sarah met with a patient named Destiny in the free clinic. The patient complained about pain, but Sarah told Destiny that she believed she had a stomach bug that would run its course. When Sarah told Destiny to come back to the ER if the pain persisted. Destiny begged Sarah to help her with her head pain. Sarah encouraged Destiny to drink liquids to rehydrate. Destiny accused Sarah of claiming that she was lying about her pain. “You don’t give a damn,” Destiny said as she walked out.

After Sarah left work, she was walking through the square when Destiny rushed up to her and asked for help. Xander noticed from across the square, and he caught Destiny as she collapsed in front of Sarah. Sarah could not find a pulse on Destiny, and she called for help.
