Blake Garrett reportedly found dead in a sober living home with a lighter and foil with burnt residue by his bed

Blake Garrett, the child actor best known for his role in How to Eat Fried Worms, has died at thirty-three years old. As per a report shared by Entertainment Weekly, Garrett passed away on Feb. 8 from acute fentanyl toxicity. The publication reviewed an autopsy report from the Oklahoma Office of the Chief Medical Examiner, which has since shed new light on the circumstances surrounding his death.

Blake Garrett was found unresponsive in his room at a sober living house in Tulsa, Okla., and was pronounced deceased at the scene. Drug paraphernalia, including rolled dollar bills, foil with burnt residue and a lighter, were recovered near his body.

The autopsy report went on to document several personal details about the former child actor. Among his tattoos were his birth year, "1992," on his neck, a tiger and a building on his right arm, stars on his right thigh, and "Mom" on his right foot, alongside a peace sign on his left foot. At the time he was found, Garrett had a yellow beaded bracelet and a white rubber bracelet on his right wrist.

As per the report shared by Entertainment Weekly, toxicology tests revealed an extremely high level of fentanyl in Garrett's blood, with his death ruled accidental from acute fentanyl toxicity. The autopsy report additionally noted diffuse cerebral edema, or swelling of the brain, as well as shingles in various stages of healing on his lower left abdomen and left thigh.

Blake Garrett's mother, Carol, had confirmed her son's passing to TMZ back in February. She revealed that Garrett had visited an emergency room in Tulsa just one week before his death, citing intense pain. He was subsequently diagnosed with shingles.

Carol also shared that her son had relocated to Tulsa three years prior to his passing with the intention of focusing on his sobriety, and that she had believed he had truly turned things around.

As per a report shared by Us Weekly, Blake Garrett's path to How to Eat Fried Worms started well before the 2006 film. Local productions like Aladdin and His Magical Lamp and Peanuts: A Charlie Brown Tribute came first. He was ten years old when he joined Barney's Colorful World International Tour, an arena show he traveled with before making his video debut in Barney's Colorful World, Live! in 2004.

Two years later, the then thirteen-year-old was cast as Plug in How to Eat Fried Worms, a friend of the film's classroom bully Joe, played by Adam Hicks. Garrett told The Oklahoman he played the bully's henchman, though he was quick to add,

"But everybody got along on the set."

One moment from filming stuck with him. A scene on a gravel road called for the kids to slide their bikes to a stop, and Garrett happened to kick up gravel that struck the camera on the ground.

"They really liked that shot, and that's the one they used in the movie," he told the outlet.

He also recalled getting first pick from a row of bikes on set. Small details, but the kind a child doesn't forget.

The film earned Garrett and his castmates a 2007 Young Artist Award for Best Young Ensemble in a Feature Film. He returned home to Edmond, Oklahoma shortly after, finished middle school at Summit, and never sought the spotlight again.
