Chicago Med Season 12 Episode 2, titled Third Rails premiered on October 14, 2026, at 8:00 PM ET on NBC. The episode will also be available to stream on Peacock the following day. Previously, in Chicago Med Season 12, Nurse Doris told Sharon Goodwin that she had leaked Dr. Theo Rabari's medical files. Miranda blamed Goodwin for the leak, and she was on the verge of losing her job. Dr. Hannah Archer told Dr. Dean Asher that she will be on maternity leave. Elsewhere, Caitlyn told Ripley that she is on a three-month sabbatical.
According to One Chicago Centre, Episode 2 of Chicago Med Season 12 picks up right after the chaotic premiere day at Gaffney Chicago Medical Centre. It begins with a glimpse of Dr. Frost's new love interest. As they shower together, they get interrupted by Ripley, who knocks on the door to ask for his toothpaste and toothbrush. Ripley tells that Naomi has moved to Portland for a new fellowship.
Archer and Asher drop off their daughter, Mabel, at a hospital daycare. Mallory Sloan tells Archer that she will be moving resources to help the E.D. Nurse Doris explains to Goodwin and her eldest son, David, that she was simply trying to save Charles. She decides to turn herself in.
Soon, multiple injured subway surfers arrive at Gaffney's E.D. Archer takes care of a victim named Nolan with severe burns. Asher looks after Samara, who has come with a neck injury. Frost and Tanaka-Reed team up to save Christian. But Nolan's condition started to worsen, and Archer sought Tanaka-Reed's help.

Chicago Med Season 12 Episode 2 recap: A subway surfer run turns chaotic
As multiple injured subway surfers are brought to Gaffney Chicago Medical Centre for treatment, the medics hurry to save them. Frost informs a victim, Christian, that he has an aortic dissection. He also adds that his brother is safe but in a critical condition. Christian blames Nolan because subway surfing was his idea.
Asher takes Abrams' help to tell Samara, another subway surfer victim, that she might not be able to walk again. Samara tells that she is not ready for the surgery, even though she knows skipping it could turn fatal. Among the injured subway surfers, unfortunately, Nolan dies after bleeding out in surgery.
Asher and Abrams use leeches on Samara's wounds in place of surgery.
Elsewhere, Ripley examines another patient called Ellen. He orders more tests for Ellen, noting her weight gain in the past three months. Ripley then consults with Dr. Charles about Ellen, considering it could be a psychological issue.
Meanwhile, Goodwin's son David advises her that allowing Doris to remain silent would complicate legal matters.
Archer confronts Sloan regarding the overflow room
Charles speaks to Ellen about her life, and she shares that she is in a custody battle with her ex over their son. She expresses her desire to get on a GLP-1, but Charles feels she is hiding something.
Archer teams up with Frost to save Christian's leg, but Archer's talk stresses Christian out. Frost calms him down. Archer then confronts Sloan about the mystery room. It turns out to be an overflow room that Sloan believes could be helpful, but Archer wants the overflow room gone. However, Sloan isn't ready to give it up.
Does Christian survive the surgery in Chicago Med Season 12 Episode 2?
According to Chicago One Centre, Frost speaks with Nolan's father, assuring Christian would be all right before the surgery. They allow Christian to meet his father before the surgery. However, Christian is unaware of Nolan's tragic fate. When Christian asks his father about Nolan, Frost lies to him, saying he is resting. Christian asks his father to tell Nolan sorry, and his father turns emotional, which causes a panic situation. Frost and Tanaka-Reed complete the surgery.
Elswhwere Samara's mother doesn't seem to agree with her daughter's self-destructive decisions. Asher tells Samara that she will be at constant risk of getting strokes. In the end, Samara agrees to have the surgery.
Ripley and Charles treat a patient with addiction in Chicago Med Season 12, Episode 2
Ripley decides to speak with Ellen's son, and he learns that she taught him how to call for help. Ripley shares this information with Charles. This instance reminds Ripley of his past and his mother's drug problem. He remembers that he was taught to do things himself in case of overdose.
Charles talks to Ellen about her alcohol consumption and whether she has an eating disorder to qualify for a GLP-1 that would help her with the addiction. Ellen shares that she was worried that her addiction might lead to losing her son's custody. Charles tells her firmly that he could prescribe GLP-1 to help with diabetes, but not for addiction. He advises her to face the problems bravely.

Goodwin takes the meeting with Miranda. She has decided to resign from her position and take on the deal. Nurse Doris shows up at Goodwin's house again. She is angry that Goodwin took the deal. Goodwin seems to have planned something with Archer. Archer apologizes to Sloan for his reaction. However, it's part of a plan that he has mapped to have Goodwin join as the new charge nurse. It seems like Sharon Goodwin isn't leaving Gaffney Chicago Medical Centre soon.
