Chicago Med Season 12 premiere: A wedding shooting leaves Gaffney in chaos- recap ahead 

NBC's medical drama, Chicago Med Season 12, premiered on October 7, 2026, with the season opener titled Shots Fired. Written by Allen MacDonald, the series will follow the chaotic lives of the medical professionals of Gaffney Chicago Medical Centre.

Chicago Med Season 11 finale ended with a bittersweet note. While Sharon Goodwin managed to save Charles' position at Gaffney, her own future was thrown into jeopardy. In the finale episode of Chicago Med Season 11, Miranda asks her to resign after accusing her of leaking Theo Rabari's medical files. Elsewhere, Hannah Asher gave birth to her daughter Mabel after a tense labour during the hospital lockdown. Archer later confessed his feelings for Asher, but the two are still not officially a couple.

Chicago Med Season 12 will feature a medical emergency involving a bride. Beverly Hills star Gabrielle Carteris guest stars in the episode as the groom's mother. Goodwin tries to defend herself while Asher makes a pivotal decision during her maternity break.

Chicago Med Season 12 Episode 1 recap: Mallory Sloan is the new Gaffney administrator

According to One Chicago Centre, the premiere of Chicago Med Season 12 picks up three months after the events of the Season 11 finale. Mallory Sloan, a new Gaffney administrator, walks into the emergency department and watches Dr. Wu revive a patient.

Elsewhere, Archer cares for a young patient who got hurt when a squishy toy exploded. Doris informs Archer that they need more support in the emergency department. Due to budget cuts, there are limited staff members.

Since Goodwin stepped down and Sloan took over, it was nothing but absolute mismanagement. When Archer asked for more staff, Sloan denied it. Charles offers to help. Elsewhere, Goodwin says that she did not leak Theo Rabari's private files. Goodwin also accuses Miranda of racism or ageism.

Asher visits the hospital with Mabel and brings food for Archer. She explains the co-parenting situation to her colleagues.

Chicago Med Season 12 premiere recap: A wedding gone wrong

A massive shooting occurs at a wedding, injuring at least a dozen people, including the bride. Archer and Wu try to save a bride, Tamar. Wu finds out that Tamar is pregnant and the bullet has injured her uterus. Dr. Tanaka-Reed steps in to help other patients. Ripley declares that a bridesmaid has already died. Archer asks for Asher's assistance, and Asher confirms that Tamar is 23 weeks pregnant and offers to lead her surgery.

Mrs. Sutherland, the groom's mother, has been worried because her son Dexter has been missing since the shooting at the wedding. Tamar wakes up suddenly, and Asher assures her that her baby will be okay. The bride tells her that her former partner, Henry, was the shooter. Tamar explains to the police that Henry couldn't accept their breakup, even after she got engaged to Dexter. Tamar eventually reveals her pregnancy to Mrs. Sutherland.

Asher asks Tamar whether she wants to deliver the baby and remove the bullet at the same time. Tamar agrees to have the surgery. Soon, Dexter arrives in the ED looking for Tamar. His mother takes him to see Tamar. However, when he walks away, Archer and Charles notice something weird. Dexter doesn't seem to have any injuries, and they wonder why he hasn't been injured in the shooting.

Charles meets with Dexter, and Dexter admits he never drove to the church as he was unsure of his decision. He switched off his phone and drove around. His mother convinced him that Tamar was marrying him for his money. Charles suggests that he talk with Tamar.

Chicago Med Season 12 Episode 1 ending explained: Frost treats Dakota, an unexpected death, and Tamar's successful surgery

Frost helps an eight-year-old patient, Dakota, whose eyes are covered with bandages. Dakota can't see anything, and Frost orders some tests to find out what happened to Dakota. Bruno, the musician at the wedding who brought in Dakota, was sent to Ripley to check on his neck injury. Bruno promises Dakota that he will be back.

Dr. Abrams and Frost perform an eye exam on Dakota and find out her eyes are okay. It turns out she is functionally blind due to a trauma response. Abrams then begins the test on Dakota's brain, and Frost encourages her to stay positive. Bruno also encourages the young girl.

Ripley brings Bruno's discharge papers. Soon after, he is fatally hit by a car that crashed through the ED's glass doors. Bruno dies on the spot.

The driver is none other than Tamar's ex-boyfriend, Henry. Ripley confronts him, and Henry goes into shock. Archer and Ripley realize that he has broken ribs and a broken clavicle. Ripley and Wu try to keep Henry alive. Ripley informs Frost that Henry has killed Bruno. He also tells him that Dakota's mother was killed.

Dexter's mother tells him that Tamar is pregnant. She tries to speak ill of Tamar's intentions. Charles asks her to leave.

Asher informs Tamar that the baby is safe and the surgery went well. Dexter apologizes to her. She is clearly hurt when Dexter admits to having second thoughts. When Dexter's mother tries to enter the room, he stops her.

Frost meets Dakota, and Ripley brings in some good news. Dakota's mother is still alive. It turns out there was a mix-up. While talking to Dakota's mother on FaceTime, Frost tries to tell them about Bruno. Dakota's mother understands Frost's hesitation.

Who leaked Theo Rabari's medical files in Chicago Med Season 12?

Asher tells Archer that she will be on maternity leave for a few weeks before resuming work. Archer addresses the ED staff and thanks them for their hard work. They also share a moment of silence for the 12 patients they lost.

Ripley video calls Lennox, who is on a three-month sabbatical. She is in Zurich for a drug trial for her GSS disease. Ripley tells her he loves her.

In the end, Nurse Doris dropped by Sharon Goodwin's office and revealed that she had leaked Theo's medical files.

New episodes of Chicago Med Season 12 drop every Wednesday at 8 PM ET on NBC and the next day on Peacock.
