Previously, when Sonya shared her pregnancy news with TJ, he seemed elated. However, in this episode, he turns increasingly distant. Elsewhere, Sonya is struggling to understand why their equation has changed all of a sudden. According to TV Insider, TJ can't sleep at Sonya's and chooses to leave while she's fast asleep. Sonya worries about the situation, and she tells Amy that TJ is detached from the situation. When Amy confronts him, he brushes off her concerns. However, a memory flash tells Amy that when TJ finds himself in a complex situation, he tries to escape.

The viewers get a glimpse of TJ struggling with PTSD. Amy remembers how TJ got disturbing nightmares, which turned so intense that once he tried to physically attack Amy in his sleep. Amy suggests that he needs to get help. Sonya is unaware of TJ's terrible flashbacks; as a result, they grow distant. According to The TV Cave, by the end of the episode, TJ tells Sonya that he needs space for the night to get some sleep.

In an interview with TV Insider, Patrick Walker, who plays TJ in Doc, agreed that TJ's coping mechanism is avoidance. Walker added,

"TJ is genuinely, in my opinion, extremely excited about having a child. It is really scary with the aspirations that he has in life. And he’s growing and excelling extremely fast. I know that he’s excited, but it’s just a little block in the road, and it’ll get worked out. But I know he’s genuinely happy about it"

The patient was previously exposed to measles via an unvaccinated relative. Amy struggles to treat the patient with a potentially fatal illness. This case triggers Amy's memories of losing her own son, Danny. As a result of these tragic flashbacks, Amy confronts her former partner, Michael, and their argument shows how their son's death continues to affect their relationship.

This is a significant decision, but it comes with multiple complications and raises questions of boundaries and expectations.

Elsewhere, Graham, the new intern, seems to believe that his privilege and connections will help him survive smoothly in Westside. However, he is likely unaware that it's not that easy. He repeatedly tells Amy that his stepmother is a huge fan of Amy. However, Amy isn't impressed with the constant buttering up.
