According to The TV Cave, Amy and Jake treat a patient with a heartbreaking past. Their patient was a woman named Sophia who worked as a drug mule. She was exploited as a teenager by men who forced her to transport drugs. She also underwent a botched procedure by someone who is not a doctor. The botched procedure has led to the medical emergency. The woman was in witness protection. It turned out Sophia hid her past from her partner, Shawn, as she thought knowing the truth might be risky for her, Shawn, and his daughter.

Jake and Michael planned to treat her on the hospital's fourth floor to keep the operation hidden from the staff and prevent word from getting to her former gang.
