Doctor on the Edge episode 3 starts with the village chief accusing Ji-ui of taking his money, revealing that the insurance company told him he has angina and not myocardial infarction, and they won't cover angina. Ji-ui tells him that when angina progresses, it leads to myocardial infarction. This leads to both arguing with each other, with Ji-ui saying that if he had given him the antacids, he would have died.

Additionally, the two also get a warning from Miss Hwang, who tells them that the chief won't back down. As expected, the village chief asks the people not to join hands with the health center and even makes the health center crew do cleanup work near the ferry point and Ji-ui, under pressure, decides to leave.

While this wasn't enough for Ji-ui, he ends up in an argument with Jang-su, who already threatened him in the previous episode. While Ji-ui tries to convince him to get proper scans on the mainland, Jang-su doesn't listen to him. Ha-ri noticed the stress Ji-ui was facing after he had handed out a full month of medicine a few days later. Ji-ui just tells her that he wants to leave after finishing his posting.

By the end of the episode, Ha-ri learns that the insurance company was trying to take advantage of the chief, and the village chief later brings seafood after learning about an insurance issue.

Chiyeon tells Ha-ri that he is afraid of dogs. Just when they were about to leave, the team worked together to save the dog after he got injured.

Meanwhile, Ji-ui, who already texted Ha-ri for the parcel, gets caught by her grandmother at her house after he slips and gets injured. Ha-ri's grandmother treats him and even saves his contact. Ji-ui starts searching for Ha-ri the next day, only for him to learn that she had taken a day off.

Ji-ui also learns about Jang-su's previous heart issue from Miss Hwang and rushes to help him and takes away his medicines. Jang-su soon realizes that Ji-ui is only helping and protecting him.

On the other hand, Ko Changmok, a government official, pushes for stricter action against the island clinic by holding a press conference.

At night, Ji-ui and Ha-ri meet and clear up the selfie confusion. Ha-ri then shows him fireflies and apologizes for not being more supportive. He also asks her why she went to Madeokdo with Chiyeon, with Ha-ri telling him that it was because of the distance.

Viewers also see Chiyeon shutting down Jucheon when he complains about the poor setup in Doctor on the Edge, saying they just have to manage with what they have.

When Chiyeon gets asked for a drink by Jucheon, he refuses because of lack of sleep. Soon, he sees messages on the group chat talking about a nurse who joined the health center, having a mole on her nose and short hair. They call her "Doctor Hunter," which he suspects is Ha-ri.
